Are there a certain type (upsampada/senior, for example) of monk who can ordain another, or can anyone be approached for this?

Only a monk authorised to be a preceptor (Upajjhaya) can do this, I'm not sure what the criteria for becoming a Upajjhaya is.

This is both true and false!
yes today certain groups, such as in the Thai sangha, Upajayas need permission, but it isn't Vinaya exactly, and I am not sure whether it is a custom of all the sanghas (i.e. burma, lao, Sri Lanka etc.)
Personally I see the authorisation as a safeguard for potential monks, that the upajjhayas meet the minimum qualification described in the texts.
This offering maybe right, or wrong, but it is one, the other, both, or neither!
