For negative applications of the term in the suttas it would be better to look at the use of its source verb, adhimuccati, and its past participle adhimutta.

As for the noun, see its use in the chapter on paticcasamuppāda in the Abhidhamma's Vibhanga and its commentary.

Does anyone know when these were first formulated and by whom? The formulation is an Indian Mahayana one. The use of the seals as a yardstick for judging whether a teaching is Buddhist seems to be Tibetan; perhaps it has an Indian precedent, but I've yet to see one. I am also curious if they can be...

For rules contained in the two Pātimokkhas you need to look in the Bhikkhuvibhanga and Bhikkhunīvibhanga.

Are particular paramis associated with particular persons of the path (i.e. stream-entrant candidate, stream-entrant, once-returner, etc. up to Arhat) in Theravada? I don't think the Pali texts ever make such a connection. I suppose it might be possible to do so inferentially on the basis of suttas...
